Your Opportunity
Provide client centered health services and case management support to our clients within the Common Ground AOD Hub. The role will actively engage to foster access to Alcohol and Other drug, health and social services in effectively addressing individual clients needs and requirements.
Your key responsibilities will be to:
- Support and provide intensive ongoing person-centred case management and care coordination within the clinical governance framework
- Provide treatment interventions in accordance with NSW National Quality Framework for Alcohol and Drug Treatment
- Adopt holistic approach to improve health outcomes and assist the client through therapeutic interventions and referrals
- Undertake initial assessments and registrations and provide assistance in transitioning clients out of the services
- Assist Program Manager and Other team members in addressing complex case and/or emergency situations
- Collaborate with both internal and external stakeholders to oversee clinical governance of the AOD hub
- Complete a range of administrative duties for the efficient running of the services
- Currently registered with AHPRA as a Registered Nurse (Division 1)
- Experience and/or demonstrated understanding of working in a clinical AOD service
- Experienced dealing with clients with complex mental health illness and challenging behaviours
- Has a sound understanding of Mission Australia’s values and code of conduct
- Well-developed written and oral communication skills, including highly developed administrative skills
