What you'll be doing
The NSLHD Nursing and Midwifery Workforce will be compassionate, competent, confident, contemporary, capable and committed to leading person-centred health care.
This position is required to work closely with patients/clients and carers and their General Practitioners (GP) to identify care needs (Clinical and non-clinical) and develop person centred care plans, strategies and goals to improve quality of life and prevent unplanned presentation to ED or hospital.
People of Aboriginal and/or Torres Strait Islander background encouraged to apply.
Candidates will need to meet the following criteria:
- Registered Nurse - Division 1 with Australian Health Practitioner Regulation Agency (AHPRA) with relevant post-registration qualifications and at least 3 years’ experience working in the clinical area of their specified post-graduate qualification, or such other qualifications or equivalent experience deemed appropriate. Current NSW Drivers Licence.
- Ability to identify and improve opportunities to enhance continuity of care between acute hospitals, primary care providers and community-based services and a comprehensive knowledge of primary health and community based services.
- Demonstrated ability to communicate effectively with patients, carers, and a diverse range of health care providers including those in government, non-government, residential aged care and community organisations.
