Where you'll be working
Ryde Hospital
Ryde Hospital is 194 bed general hospital providing acute services including emergency medicine, general and sub-specialty medicine, and general and subspecialty surgery.
Subacute admitted rehabilitation is provided in the purpose built Graythwaite Rehabilitation Centre.
The Intensive Care Units has a role delineation of level 4 and has coronary care beds included.
Ryde Hospital is having an incredible $479 million complete hospital redevelopment that will transform the delivery of healthcare for the Ryde local community.
The Ryde Hospital Redevelopment will result in new health facilities and enhanced services including emergency, ICU/critical care, inpatient units, outpatient/ambulatory care services and much more.
Construction commenced in early 2023, and will provide you with a wonderful and exciting opportunity to be part of the Ryde team to change the future health services for the Ryde Community completely.
What you'll be doing
The NSLHD Nursing and Midwifery Workforce will be compassionate, competent, confident, contemporary, capable and committed to leading person-centred health care.
- The Clinical Nurse SpecialistDischarge Liaison is responsible for ensuring patients are assessed and manage for admission and discharge with plan in place.
- Refer patient to services as appropriate.
- Managing workload and competing priorities.
- Maintaining good communication with multidisciplinary teams across disciplines and with providers of health care in the community.
Candidates will need to meet the following criteria:
- Division 1 Registered Nurse with post registration qualification relevant to speciality and at least 3 years experience working in the clinical area of their post graduate qualification or such other qualifications or relevant equivalent experience.
- Evidence of and commitment to patient/customer focused care, clinical governance and quality improvement
- Evidence of advanced level of knowledge and skill related to management of speciality area
- Demonstrated ability to work well within a multidisciplinary team, with an emphasis on service delivery, quality of care and team cohesion.
