Our client is seeking a compassionate and dedicated Clinical Team Leader to join their team at the Port Macquarie aged care facility on a permanent full-time basis.
In this role, you will play a vital role in ensuring the well-being of our residents by providing clinical and holistic case management.
Key Responsibilities:
- Provide clinical leadership and direction to a care team, ensuring adherence to best practices and resident care protocols.
- Support the Clinical Care Manager with case management for a designated cohort of residents.
- Collaborate with residents, their representatives, and the care team to develop and implement individualized care plans that meet each resident's unique needs and goals.
- Identify and manage risks to resident safety and well-being.
- Work collaboratively with the Clinical Practice and Quality and Compliance team to deliver best possible outcomes for residents.
- Respond to changes in resident status in a timely manner, including reassessing and adjusting the care plan as required, and engaging other healthcare providers and specialists as needed
- Assist in implementing and documenting quality improvement activities.
- Identify areas for improvement in resident care practices and develop action plans.
- Ensure all clinical incidents and resident feedback are captured and addressed promptly.
- Coach and mentor care team, fostering their professional development and skillsets.
- Facilitate training sessions and on-the-job coaching to enhance clinical knowledge and skills.
Requirements
- Must be an AHPRA Registered Nurse with current registration.
- Postgraduate qualification in Gerontology, Dementia Care, Palliative Care, Management highly regarded.
- Minimum 3 years postgraduate experience in a relevant environment.
- A sound understanding of the Aged Care Quality Standards.
- A sound understanding of person centred and relationship-based care principles
- National Police Clearance Check
- Australian Working Rights
- Must be willing to work weekends to support continuity of care and Team Leadership.
Desirable
- Previous experience in gerontology/aged care nursing at a senior level.
- Demonstrated compassion, tolerance, patience and empathy for aged people.
- An understanding of Veterans’ experiences and needs.
