ACCP are currently looking for an amazing Clinical Team Leader NUM 1 to join our team/family. The Clinical Team Leader oversees the clinical functions of the SWSLHD Aboriginal Chronic Care Program (ACCP) to ensure the provision of care coordination services that are evidence based, holistic and responsive to the specific needs of Aboriginal clients and their families.The Clinical Team Leader will work collaboratively with the Aboriginal Chronic Care Program Manager to ensure the ACCP functions within a strong clinical governance framework that is culturally responsive and that the multidisciplinary team is supported to deliver effective service.The ACCP team is a small but mighty multidisciplinary team made up of Aboriginal Health Workers/Practioners, Care coordinators, Dietitians, Admin, CNC’s, Clinical Team Leader, Facility Manager and ACCP Program Manager.The team covers the whole SWSLHD with 3 office/clinic sites across the district – Wellama (Bankstown), Budyari (Miller) and ACCP offices (Miller).The team has a big focus on not only the wellbeing of our clients but also each other.The team work collaboratively in the provision of specialist services to Aboriginal patients and their families with chronic and/or complex needs in a culturally safe, patient centred way to facilitate optimal health outcomes.The position aims to ‘Close the Gap’ in health disparities for Aboriginal people by addressing their chronic and complex health conditions and contributing factors.What You'll be Doing

  • Demonstrated understanding of culturally safe health service models for Aboriginal health communities and experience delivering culturally safe health services to Aboriginal communities.
  • Relevant postgraduate qualifications and/or management experience or equivalent work experience or a combination of study and experience deemed appropriate by SWSLHD.
  • Demonstrated senior nursing experience with experience in leading staff and managing services in a relevant setting.
  • Proven ability to ensure the human, physical and financial resources of the unit are managed to provide safe, compassionate, quality care that is culturally responsive.
  • Demonstrated ability to effectively communicate and engage with patients/consumers, carers and the multidisciplinary health care team to identify and ensure responsiveness of care needs.
  • Demonstrated capacity to interact with others in a personable and professional manner and manage conflicts and performance issues when they arise.
  • Demonstrated evidence of the use of critical thinking as it relates to the management of the unit and the delivery of evidenced based nursing care.
  • Demonstrated understanding and participation in risk management, work health and safety and quality improvement in the workplace.
  • Must possess a valid Australian Drivers Licence (excluding learner driver's licences). If not already in possession, a NSW Drivers Licence must be obtained within 3 months of appointment.
