THE ROLE
We have an opportunity for a Community Coordinatorto join our team on a full-time basis. Our Community Coordinators are strongly committed to supporting both our clients and each other to thrive.
The successful candidate will be required to support our prospective and current senior clients in navigating the full cycle of their care package. From initial meeting through to navigating the My Aged Care system, you will work with your assigned clients to develop goals that promote choice, control, and independence. These goals drive development of individualised budgets and support plans, to allow our clients to thrive in their own homes.
You will conduct regular in person reviews with clients, which will allow you to build genuine relationships that help you understand and assess current and future needs, ensuring maximum support and prompt identification of changes in care requirements.
While the role is team-based at our centrally located office at Park Road - Milton(4064), you will work autonomously and be on the go day-to-day, visiting your clients and supporting our team of Nurses and Carers out in the community. You will cover the Brisbane bayside, Logan area and onwards towards the Gold Coast, using a company vehicle. (There may be a requirement to travel to other locations based on business need, so flexibility is important).
Therefore, this role will be best suited to a passionate Registered Nurse, who is able to thrive both autonomously and within a team, and who lives at or around the Gold Coast or Logan area.
Training will be provided to the successful candidate on the “My Aged Care” package and associated processes.

To be successful in this position you will be someone who has:
- A genuine warm and friendly disposition, with a passion for enhancing the quality of life, dignity and independence of Seniors/Aged clients in the community.
- Excellent interpersonal and communication skills, including time management and attention to detail.
- Ability to thrive autonomously and also as part of a collaborative and supportive team.
- Current AHPRA registration - Registered Nurses only.
- Current First Aid and CPR certificate/s.
- Evidence of Influenza (2024 formulation) and Covid-19 (minimum of x2 doses) vaccination.
- Australian or New Zealand citizenship, permanent residence or valid working visa
- A current and valid OPEN Australian drivers licence.
- A willingness and flexibility to travel to client locations day-to-day, using company vehicle.
