An exciting opportunity to join us in the Monash Health Community Youth and Family Team, Forensic Mental Health in Community Health (FMHiCH).
The Youth and Family Team, is comprised of numerous disciplines that focus on the assessment and delivery of youth and family-focused care. The team is committed to working with vulnerable and marginalised young people and adults by providing high-quality, multidisciplinary programs which embrace the client-centred practice and evidence-based services. We operate from a person - centred care framework and strongly align with the social model of health. FMHiCH provides clinical services to clients on Community Corrections Orders or parole who have a mental health condition on that order or parole.
A Day In The Life
This is a Community Development role within a multidisciplinary team (social workers, psychiatric nurses, general practitioners, psychiatrist, psychologist, exercise physiologist and Community Development Workers)
Working in line with Monash guidelines and procedure, the role of the Community Development Worker is to undertake a holistic assessment of needs, supporting the wider team to run group programs, treatment planning, monitoring, advocacy, and linking of the service user with appropriate community support services. You will see a wide variety of clinical presentations and levels of acuity.
You will enjoy the challenges of working with the complexities of mental health to improve individual quality of life and to make a positive difference every day. You will work closely with a range of mental health and health services to effectively co-ordinate client care. This opportunity will see you enjoy a balance of working autonomously and as part of a team, with access to high quality clinical supervision being offered.

What you need:
  • Experience in a community service or health setting
  • Experience in supporting groups is desirable
  • Experience working in Mental Health/Forensic settings is desirable
