ABOUT THE ROLE:
Are you passionate about Community Engagement and increasing people's understanding of palliative care? Do you enjoy working with other service providers to ensure an integrated response to patient and family palliative care support needs?
- Permanent role, 32 hours per fortnight
- Community development and engagement with targeted community groups
- Develop collaborative relationships with services to increase understanding of palliative care & end of life care issues and develop coordinated care and referral pathways
- Integral member of multi-disciplinary team caring for palliative care patients in their home
ABOUT YOU:
- Tertiary qualification in either community development, health, social science or health policy
- Experience in community development & engagement
- Experience in palliative care and/or community health is desirable
- Current unrestricted driver’s license
- Able to work at Sunshine (main) & Parkville & Werribee office locations as required

Mercy Palliative Care is a community palliative care service operating in the Western region of Melbourne and provides a holistic multidisciplinary approach to palliative care. We provide psychological and spiritual support, caring for people of all ages with life-limiting illnesses in their homes as well as complex symptom management. We currently care for over 500 patients across the region in their own homes.
