- Community Health Nurse- Caulfield Community Health Service
- Permanent Part time (64 hours per fortnight)
- Salary Range: $102,000 Pro Rata
- Location: Caulfield Hospital -Caulfield Community Health Service
The Caulfield Community Health Service (CCHS) provides a range of clinical and health promotion services for people of all ages who live, work or study within the local community. These multi-disciplinary services are provided in the centre, community, home or via Telehealth. CCHS operates from the Caulfield Hospital site.
Position Summary
The Community Health Nurse will sit within the Adult Health team and you will:
- The provision of flexible, consumer-driven nursing services for CCHS clients, their carers and families within a social model of health.
- Conducting comprehensive nursing assessments, management, education, referral and discharge.
- Using nursing knowledge and skills to effectively engage and build rapport with clients and their families, in order to advocate for their health needs, and to facilitate their health literacy skills
- Work within a multi-disciplinary team to ensure a holistic approach to client support
- Liaise with relevant agencies to coordinate, facilitate and communicate client treatment and outcomes. This could include local councils, ACAS, My Aged Care and relevant service providers.
- Plan and manage own workload to ensure adequate time for the achievement of primary objectives of the position
- Degree in Division 1 Nursing
- Registration with the Australian Health Practioner Regulation Agency (AHPRA)
- Experience and skills in a range of nursing specialities relevant to the community health sector
- Excellent communication and interpersonal skills with thought to Health Literacy across diverse community members
- Current Victorian drivers licence
