Job Description: We are currently seeking a compassionate and skilled Community Nurse to join our team in Hervey Bay. As a Community Nurse, you will play a vital role in providing clinical care and support to individuals with disabilities within their own homes and community settings.
Responsibilities:
- Conduct comprehensive assessments of clients' health needs and develop individualized care plans.
- Provide skilled nursing care, including wound care, medication management, and administration of treatments as prescribed.
- Monitor and evaluate clients' health status and response to treatment.
- Collaborate with multidisciplinary teams, including allied health professionals and support workers, to ensure holistic care delivery.
- Educate and support clients and their families in managing health conditions and promoting wellness.
- Maintain accurate and timely documentation of nursing interventions and client progress.
Requirements:
- Registered Nurse qualification with current registration with the Australian Health Practitioner Regulation Agency (AHPRA).
- Experience in community nursing or a related field is highly desirable.
- Excellent clinical assessment and critical thinking skills.
- Strong communication and interpersonal skills, with the ability to build rapport with clients and their support networks.
- Ability to work independently and as part of a collaborative team.
- Commitment to upholding the values of integrity, respect, and dignity in care delivery.
- Current driver's license and access to a reliable vehicle.
