Your opportunity
The Community Health Nurse is a clinical nurse who contributes to the provision of high-level community nursing support through care coordination, planning and support for clients with multiple co-morbidities and their carers across the continuum of care.
You will provide leadership in clinical decision-making to give and/or coordinate care to the clients and their carers/ families within our community (St George and surrounds) to ensure timely access to healthcare services in the community.
