Connecting2Community Support Worker

Connecting2community (C2C) is a co-designed model of psychosocial support delivered by peer workers supported by and connected to an established multidisciplinary team of mental health clinicians. The clients of Connecting2community may include some forensic clients but most will be from the broader community.

The C2C enables clients to identify goals and their assigned peer worker can link the client to relevant services and activities in the community, including neighbourhood centres. These goals may relate to social skills and friendships, managing daily living, budgeting skills, finding or maintaining a home, volunteering, education, physical health and building confidence and resilience. The model provides a time-limited intervention and is underpinned by the promotion of recovery and client choice and control. Eligible clients are adults with mental illness who are not eligible for the NDIS or are waiting for an access decision and their NDIS plan to begin. 

The service aims to:  

• provide a comprehensive range of evidence informed psychosocial interventions that reduce the impact of mental illness and support improved functioning and contributing lives. 
• optimise the delivery, coordination, and integration of psychosocial services within the existing treatment and support service systems; and 
• deliver contemporary person-centred outcome focused services.

Salary packaging

Grampians Community Health has generous salary packaging benefits up to $15,900 which increases your take home pay. This includes personal expenses, meal and entertainment expenses in addition to salary and access to ATO Remote Housing Allowance (for eligible place of residence). Working from home

Grampians Community Health is progressive with offering the capacity for many of our staff to work remotely from their homes where the service delivery allows for it.  Access to information technology is provided to allow you the flexibility of working from home while remaining connected to Grampians Community Health staff, supervision and management.

Professional development

Grampians Community Health values our employees by supporting professional development to develop the knowledge and skills of our staff to better meet the needs of our clients.  Clinical supervision is provided for all staff who require it for their professional practice.

Purchased leave

Health professionals who are employees of Grampians Community Health may be eligible for funding of up to $10,000 per year to assist with the costs associated with their Continuing Professional Development activities. Funding is available through the Health Workforce Scholarship Program administered in Victoria by RWAV.
