Our client is a well-experienced Home Care (aged care) organisation who is seeking a person with current experience coordinatingHome Care Packages.

About the Role:
The Coordinator (Home Care Packages) is responsible for the coordination, care planning and monitoring of HCP clients to ensure a quality service is provided. You will enjoy building good relationships with clients, their families and other health professionals. This position works closely with the other HCP Coordinators and the clinical team.
This is an full time office based role, with company vehicles available to visit clients.

To be successful in this role you will have:
- Minimum 1 years experience as a HCP Coordinator
- Diploma of Community Services or equivalent qualification
- Current First Aid and CPR
- Demonstrated experience in all aspects of Home Care package coordination including creating and monitoring care plans and budgets
- Strong organisational skills including the ability to problem solve, prioritise and multi-task
- Excellent communication and interpersonal skills with the ability to work well with others in a small team environment
- Strong computer skills including MS Office (word, excel) and care databases
You will need to be a permanent Australian resident, have a National Police Check (willingness to obtain), open Australian Drivers Licence. You do need to be fully vaccinated - COVID19 and Influenza.
