The Digital Health and Quality Improvement Officer is responsible for providing support to health professionals and Residential Aged Care Homes (RACHs) across the Central and Eastern Sydney PHN (CESPHN) region, on various Digital Health initiatives.
Key Responsibilities
- Undertake work on Digital Health and Quality Improvement initiatives such as My Health Record, Digital Health Practice Incentive Payments, and data Quality Improvement (QI) activities.
- Provide client visits, meetings and telephone support to medical specialists, general practices, allied health professionals, RACHs, universities and non-government organisations on Digital Health and QI related activities.
- Develop and implement targeted, appropriate, and sustainable educational activities to address Digital Health for RACHs and other healthcare providers.
- Work with team members to support general practices around person centered approaches taking into account capacity and capability requirements and tailoring support to practices.
- Ensure timely input of all required detail into CESPHN’s Salesforce base system including relevant interactions with stakeholders.
Selection criteria – essential
- Tertiary qualifications in health, health promotion and / or relevant experience in Digital Health
- IT skills and knowledge in the use of data bases, Microsoft Office suite, data security and clinical software used by health care providers and Aged Care facilities
- Demonstrated ability to work autonomously, set project goals, prioritise tasks and troubleshoot in order to achieve key objectives within designated timeframes
- Ability to build, develop and sustain positive partnerships and productive working relationships with all levels of stakeholders, including individuals from diverse cultural backgrounds
- Experience providing training and support to health care professionals and community members
- Access to a motor vehicle and possession of a current NSW drivers’ licence
- High level of interpersonal, communication and organisational skills, including excellent phone manner
- Ability to work independently and in a team.
