Follow the link to find out what its like to live, work and play in our wonderful region:About the role:An opportunity exists for a suitably qualified and experienced Registered Nurse to join the Home Nursing Service (HNS) team as a Clinical Coordinator. The Clinical Coordinator works within a leadership role, ensuring the timely delivery of professional, holistic and high-quality nursing care within the HNS -District Nursing Service, in accordance with the appropriate program guidelines.The role focuses on the supervision and direction of staff involved in care delivery and care coordination, and the development of domiciliary nursing care as a core component of essential primary health care. The Clinical coordinator ensure all referrals are triaged according to priority of care of Home Nursing, manages a waitlist of Home Nursing Services and provides clinical resource, clinical advisory and ongoing review of the effectiveness of current processes for the Home Nursing Services. The Clinical coordinator supports a team of highly qualified district nurses and maintains networks with other Nursing Services and Allied health clinician/programs within the region.The successful applicant will have:
- Current registration with the Australian Health Practitioner Regulation Agency as a Registered Nurse.
- Broad post graduate clinical experience with a minimum 5 years post graduate nursing experience.
- Demonstrated ability in working collaboratively with multidisciplinary health professionals in the delivery, coordination of primary and acute health care.
- Proven ability to incorporate data collection and information management.
- Current Victorian Drivers Licence.
- Demonstrated expert clinical leadership, experience and knowledge related to health care.
- Computer literacy at intermediate level.
- Awareness of broader community health service within the region and experience in referral processes.
- Previous management experience at Clinical Coordinator or Associate Nurse Unit Manager.
- Knowledge of government policy relating to Community Services and Primary Care Partnerships.
- Post Graduate education in an approved program of study that supports clinical decision making(i.e. Gerontology, Diabetes Education, Stomal Therapy, Chronic Disease Management and Wound care).
