Role description
The Dhelkaya @Home Program is a new service that has been established to lead and coordinate care options for patients to receive appropriate levels of clinical care within their own homes, when safe to do so. With a focus on quality multi-disciplinary care provision based on client need and individual care planning, the ultimate aim is to offer an alternative to hospital-based care with care in the home or place of residence.
The @ Home Registered Nurse will deliver quality nursing care to our in-home clients via programs such as HITH, GEM@Home and Residential In Reach (RiR). These programs will expand to over 7 days per week. The Registered Nurse is involved in the provision of expert nursing care to allocated clients, advocating for clients providing health education, promoting health, assisting clients with goal achievement and working with other health care providers to ensure coordinated care for clients.
About you
- Bachelor of Nursing with current AHPRA registration and at least 2 years full time equivalent working as registered nurse
- Current Victorian drivers' licence
- Sound understanding and experience in delivery of acute and sub-acute nursing care pathways across both inpatient and community settings
- High level communication and interpersonal skills with the ability to establish positive working relationships with clients, carers and colleagues
