The overall purpose of this role is to identify, initiate and collaborate in the development and provision of programs to improve the delivery of quality end of life care to persons with a life limiting illness in the Far West Local Health District. The role is focused on integrating care for the person at end of life by developing the skills and knowledge base of staff, the patients and their carers.
- Lead, participate and collaborate with other key stakeholders in the design and implementation of continuous improvement activities and models of care for enhanced service provision
- Contribute to the development of specialised education for complex clinical issues impacting on patients/clients/carers and the community
- Participate on District and State working parties as required to represent the health service and area of specialty, providing feedback to relevant bodies
- Adapt and apply relevant scientific research to clinical practice within services to facilitate delivery of current evidence based clinical practice for patients/clients
- The Clinical Nurse Consultant provides expert clinical advice to patients, carers and other health care professionals within a defined specialty. The Clinical Nurse Consultant develops, facilitates implementation and evaluates care management plans for patients with complex health needs
