Summary
We are looking for an experienced (specialist) Support Coordinator in the Perth Metro area. It is a flexible job where every day is different.
Some of your daily tasks include:
- Helping participants understand their NDIS plan
- Working with participants to plan and coordinate their supports
- Connecting participants with supports and other services
- Establishing and maintaining supports
- Setting up service agreements and service bookings
- Coordinating with participants, their support workers and their family members to help participants exercise choice over their supports
- Supporting participants in finding ways to be more independent

Requirements
- Minimum 2 years of demonstrated experience as a (specialist) Support Coordinator with the NDIS
- Resilient and positive outlook
- Australian driver’s licence
- NDIS worker screening check
- Full Australian working rights
