About the Role
As a Support Coordinator for Connections Inc you will provide a vitally important service to NDIS Participants. This is to improve their quality of life by collaboratively establishing good quality support arrangements that assist the Participant to meet their NDIS goals. You will also oversee these supports and maintain a positive and constructive relationship with the Participant and relevant stakeholders including Allied Health, Plan Managers the OPG, Public Trustee and Dept of Health as required. You will complete timely reports to the NDIS registration and facilitate effective Plan reviews.
We can provide you with supervision and we have the systems in place for you to function successfully in the role.
Personal Attributes
- Ability to build effective rapport participants, their families and relevant service providers to develop and implement intervention strategies in consultation with individuals and families.
- Skills in providing professional information to people with disability and their support network based on their needs and Plan goals.
- Ensuring support plans, notes and documents are recorded in line with the NDIS framework and with a high level of care and accuracy.
- Be available to provide support in Hervey Bay and Maryborough areas
- Strong communication, IT, admin and organisation skills as you will be working with billable time
Additional requirements:
- Right to Work in Australia i.e., Australian/NZ Citizen, Permanent Resident or Visa with permission to work - excludes visas with Temporary Work Rights
- A current Blue Card and NDIS Worker's Screening Check, or the willingness to gain these clearances if successful
- A current and valid Driver’s Licence
- Your own vehicle
- Willingness to undertake a "Fit2Work" Police Check
