Role Responsibilities
- Clients accessing the service are provided with a welcoming and responsive initial contact and urgent and immediate needs are addressed
- In consultation with more senior worker/s appropriate preventative action is taken in relation to potential high risk situations
- Information is provided to assist in assessing and addressing client issues, needs and circumstances
- Effective relationships are established with clients that encourage self-determination and participation and support them to improve their situation
- As directed, support is provided to clients to implement case plans in accordance with TSA case management models and standards of practice
- Clients are actively encouraged to engage in the implementation of case plans
- Clients are assisted to access support services
- Activities are monitored and clearly communicated to relevant staff for handover
- Accurate, up to date client records and data is maintained for all clients in accordance with Organisation standards and procedures
- Risks to clients, staff and Organisation are identified and bought to the attention of the Team Leader as soon as possible
- Appropriate preventative action is taken to address workplace hazards as per Organisation procedure
- Incidents are reported in the required format/s to the Team Leader as per Organisation and funding body procedures
- Service gaps are identified and recommendations for service delivery improvements are made to the Team Leader
- Certificate IV qualification in social work, welfare, community development or related field OR demonstrated equivalent knowledge and skills attained through previous appointments, service and study/development
- Relevant experience in a social service environment -DV preferable
