- Delivering crucial treatments and procedures, ensuring precision in medication administration, nutrition, hydration, and proficient pressure care management.
- Conducting comprehensive evaluations of residents' clinical requirements, encompassing vital signs and pertinent health indicators.
- Offering expert guidance on wound management and implementing strategies to optimize care.
- Effectively overseeing diabetes and other chronic conditions, emphasizing personalized and efficient interventions.
- Deploying strategies for behavioral management to elevate residents' well-being and enhance their comfort.
- Swiftly identifying and documenting changes in residents' health status, facilitating timely interventions.
- Establishing robust communication channels with residents, families, and healthcare practitioners to address evolving needs.
- Adhering to legislation, regulations, and guidelines, making informed decisions within professional practice.
- Maintaining meticulous documentation of assessments, plans, and actions to ensure accurate and comprehensive records.
- Advocating for residents with a commitment to autonomy, fostering professional relationships, and nurturing a culture of safety and continuous learning for person-centered care.
- Over a year of clinical experience with a commitment to person-centered care.
- Skilled in holistic assessments, care plan development, and compliance with regulations.
- Strong interpersonal and written communication skills.
- Proficient in computer use and effective time management.
- Knowledgeable in Workplace Health & Safety, infection control, and manual handling.
- Experienced in leading and managing staff in a clinical care setting.
- Familiar with Aged Care or similar environments.
