Job Description:
We are currently seeking a motivated and friendly practice nurse who has a genuine interest in chronic disease management as this is a large part of General practice services. If you have a warm, friendly, patient centred team approach, have excellent written and communication skills, can multitask effectively, and are passionate about primary care nursing we would love to hear from you.
This position would also suit a grad RN who has a passion for primary care nursing.
Training and mentoring opportunities are available for the right candidate.
Key Criteria:
- Current AHPRA registration is required.
- Expertise in developing and managing GP Management Plans (GPMP) and Team Care Arrangements (TCA) is an advantage.
- A strong commitment to delivering patient-centred care within our community.
- Wound management
- Immunisation and cold chain management
- Experience with Best Practice software is an advantage.
- Excellent organizational and time management skills
