We provide a range of emergency, surgical, medical and general healthcare services, including maternity, palliative care, mental health, drug and alcohol, residential care, community health and state-wide specialist services to people and communities that are diverse in culture, age, socio-economic status, population and healthcare needs.About the roleThe Community Rehabilitation Program is a short term goal based program assisting clients recovering from a recent health event/illness. Clients are seen both within their homes and centres based across the Eastern Health region. Interdisciplinary team members work collaboratively to develop care plans and client centred goals to increase patient's independence and return to optimal functioning within the community. The social work role is varied and complex throughout this process leading the team in patient's adjustment to illness, crisis intervention, resource and service provision.About youTo be successful in the role you will have:

• Degree qualification in Social Work that has been approved by the Australian Association of Social Workers for membership as a Social Worker. Maintains compliance with the AASW Continuing Professional Development Policy in order to meet the Eastern Health Allied Health credentialing standard.
• Excellent assessment and intervention skills.
• Demonstrated ability to work both independently and as part of a multidisciplinary team and to contribute to a positive team environment.
• Demonstrated communication (verbal and written) and interpersonal skills, including the ability to function as an effective member of a multidisciplinary team.
• Ability to supervise Social Work Students and Grade 1 staff when needed.
• Compliance with AASW Code of Ethics and legal requirements of the Social Work profession.
• Hold a current Victorian driver’s licence.
