About the Role
The Groups and Transport Client Intake & Support Officer is responsible for supporting clients who are accessing a range of Group Activities and Transport services funded under the Commonwealth Home Support Program, Home Care Packages, Community Transport and National Disability Insurance Scheme.
As the Principal Contact Officer for all clients within the region for the identified programs, the Groups and Transport Intake Officer will be responsible for key activities which include:
- First point of contact for potential clients providing information, support and appropriate referral.
- Supporting clients through third party referral processes (My Aged Care)
- Home visits to complete intakes and risk assessments
- Completing service agreements
- Completing onboarding of clients to relevant systems
- Home visits to complete client reviews
- Program promotion to ensure new clients engage in services for program viability
What We're Looking For:
- Previous experience in a high-volume customer service role;
- Current or recent experience in the aged care and community sector (desirable); and
- Current Driver’s license (QLD)
In addition to the above, you will need to demonstrate:
- Strong communication skills, both verbal and written;
- Ability to work in a high volume and fast paced work environment;
- Accurate data entry and time management skills;
- Sound computer skills, Confident and competent in using a range of Microsoft and computer applications to complete administrative tasks of the role; and
- High level of customer service.
