Role
We are seeking to employ a Home Care Package (HCP) Service Manager (1.0 FTE) to assist Accept Care in accomplishing our Mission, while ensuring that our Aged Care clients receive the appropriate care in accordance with the Aged Care Quality Standards. Collaborating closely with General Managers, you will take the lead in guiding and supporting a team of healthcare professionals. This involves managing within budget constraints and proactively cultivating connections with clients and their families.
Responsibilities include:
- Motivate and empower staff through coaching and mentoring
- Delegate and provide clear direction
- Lead from the fore and build a positive culture
- Oversee HCP Service operations and ensure legislative compliance
- Manage risk and continuous improvement
- Providing team with clear direction and delegating work appropriately
- Leading the team and building a positive team culture
- Oversee operations of the HCP Service and ensuring they are delivered in a manner consistent with all legislative and operational requirements
- Report to the General Managers and the Board
- Review and update process and policy
- Strategy and budget development and managment
- Support the growth of the Clinical Nursing Team; increase service hours delivery and offerings.
Essential:
- Tertiary qualification in Management, Health or a related discipline; and/or relevant experience in Home Care Package or Community Care or Aged Care Management
- Experience in multidisciplinary approach to assessment and care planning in a home setting
- Strong understanding and knowledge of Australian Aged Care Standards, National Service Standards and Home Care Package guidelines.
- High level leadership and relationship management skills
- Excellence in customer service and administration
- Full computer literacy and MS Office suite skills
- SA drivers licence and car
- Ability to gain DHS NDIS and WWCC
- Commitment to the rights of elderly and people living with disability to live dignified lives.
