About the position
We are seeking Care Navigators to join our Head to Health team located in Wagga Wagga.
The Care Navigator is responsible for connecting consumers with services and supports in the community, facilitating person-centred and integrated care.
This position delivers practical support through information sharing and education, identifying and addressing barriers to access, and facilitating warm referrals, ensuring consumers are linked to the right service at the right time. This position also works collaboratively with the other members of the Head to Health team and external service providers to maximise positive outcomes for consumers. The successful candidates will have excellent communication skills and a solid understanding of services available in the local community and online.
There are multiple positions, available in Wagga Wagga. These are available as full-time or part-time opportunities, fixed term until 30 June 2026 with possibility of extension.

We are looking for someone with:
- Tertiary qualification and/or extensive experience in a relevant discipline including mental health, health, community services or social sciences.
- Demonstrated understanding of the health and wellbeing needs of consumers with mental health and/or substance use problems.
- Demonstrated understanding of supports and services available in the local and regional communities.
- Demonstrated ability to engage with consumers and their families or carers to provide care navigation and referral support.
