Based in Gippsland, Latrobe Regional Health are looking for a
Care Coordinator to join our Health Independence Program on a part time, fixed term basis.Our Health Independence Program helps clients manage their chronic disease in a proactive and planned model.The
Care Coordinator is responsible for identifying clients and working with them to coordinate the management of their chronic conditions. Working as part of a multidisciplinary team, along with external health care providers to coordinate chronic disease management.LRH employees enjoy access to:

We are looking for candidates with:- Minimum requirement - 5 years experience in acute nursing
- Experience in respiratory and cardiac health
- Excellent interpersonal skills and the ability to work well as part of a team
