The Shine Centre is an Allied Health Private Practice and Disability Service that strives to offer high quality services to people in the Ballarat and Bacchus Marsh regions and surrounds.
We are a team of Support Coordinators, Occupational Therapists, Speech Pathologists, Behaviour Support Practitioners, Therapy Assistants and Psychologists who are passionate about working with children, their families, and the greater community.
Our clinics have onsite parking and are walking distance to shops.
About the Role
This is a part-time position that can be performed during school hours or 3 days a week.
The Support Coordinator is responsible for the following:
- Working with clients (i.e., NDIS participants / participant’s representatives) to achieve their goals in line with their NDIS plan
- Providing ongoing support coordination services to clients in line with their NDIS plan
- Using a coaching approach to assist clients in capacity building
- Maintaining accurate and up-to-date clinical notes on participant progress including any communication notes regarding phone calls or emails
- Ensuring Work Health and Safety guidelines are adhered to
- Engaging with patients, their supports and Third-party organisations in a professional manner
- Undertaking work in line with the organisation's policies, procedures and values.
- Complying with all relevant legislation and codes of conduct
- Maintaining an up-to-date knowledge of the NDIS systems and practices
- Maintaining clear professional boundaries with patients
- Maintaining an up-to-date knowledge of local services and organisations patients can utilise
- Effectively communicate and maintain relationships with the NDIA and Local Area Coordinators
About You
You have:
- Excellent communication across all domains (verbal, receptive, and written)
- NDIS and Support Coordination experience is preferred
- Excellent time management and organisation skills
- the ability to work independently and as a team
- Great technical ability and typing ability
- Disability experience (work or lived)
