Home Based Cancer Care Project Worker

Part Time – Limited tenure (6 months from Commencement)

We are seeking an experienced, positive and motivated Registered Nurse or Allied Health Professional to join our team.  Reporting to the Nurse Unit Manager Acute this role will be responsible for developing a model of care for the implementation of Home Based Cancer Care services within Colac Area Health’s (CAH) capability and in accordance with DHHS guidelines and funding requirements.

This role will review current cancer services within Colac Area Health and the broader region.  Identify opportunities for service development for Home Based Cancer Care, potential funding streams and opportunities for regional partnerships.

As our ideal candidate you:

Have previous project management experience

• Are a Registered Nurse or Allied Health Professional with current AHPRA registration
• Have a well-developed knowledge of current clinical best practice, preferably in relation to Cancer Care and/or Home Based care services
• Have a demonstrated ability to lead a project with sound networking and communication strategies.
• Have a High level investigation and research skills to determine appropriate model of care and clinic requirements
• Have the ability to establish required relationships and create referral pathways
• Have a proven ability to work well independently with minimal supervision and possess self-motivation
• Have a demonstrated ability to communicate effectively with a wide range of people
• Well-developed administration and computer skills, including project reporting.
