Your new role:The Royal Melbourne Hospital has an opportunity for a Registered Nurse - Case Manager to join the multidisciplinary TCP
Home Based team. The position is based at the Royal Park Campus. The role of the
Transition Care Program (TCP) case manager is to provide short term case management to older adults following their discharge from hospital to home. The program aims to provide additional supports and services with the transition from hospital to home and support clients to transition to longer term care arrangements.The role involve: managing a case load of complex clients, undertaking assessments and risk management, liaising with brokered service providers to meet the client’s needs, as well as education to clients and their families.
About You:- Demonstrate sound clinical skills and knowledge and be able to adapt these skills to problem solve and address client’s needs.
- Demonstrate an understanding of goal centred care.
- Demonstrate a good understanding of community programs, both within RMH and external
- Demonstrated experience in case management
- Utilise excellent verbal and written communication skills to communicate with patients, carers, other health professionals and services providers
- Use your previous knowledge and experience to problem solve and be solution focused
- Demonstrate competency in Microsoft Office, also previous use in anelectronic medical record would be advantages.
