Let's work together towards maximising capabilities and provide support to achieve operational excellence!Position Overview:The successful Homes and Living Coordinator is responsible for providing coordination and services to clients living in supported accommodation that require support within the community, including Community Access, Home and Living.The role will work closely with the Senior Manager Community Support Services ensuring consultation in a timely manner regarding potential or significant issues or risks.Supported Living, the Homes & Living Coordinator, will monitor programs and participants’ care, encouraging participant choice, delivering best practice support, and adhere to the NDIS Quality Safeguarding Framework. You will lead and develop teams of Support Workers to enable participants to live as independently as possible, in accordance with their needs and choices. This care will be delivered in the supported independent living (SIL) facilities.What do we need from you:
- Past success in coordinating and leading a team in the community disability sector.
- Demonstrated knowledge and understanding of the National Disability Insurance Scheme, Supported Independent Living and compliance regulations.
- Previous experience in a Leadership role with the ability to manage complex matters.
- Excellent interpersonal skills and building positive relationships with key stakeholders.
- Strong written and verbal communication skills, including development and analysis of reports.
- Excellent organisational skills with the ability to achieve deadlines in a fast-paced environment.
- Excellent analytical and problem-solving skills.
- Demonstrated strong IT skills, including the ability to use portals, systems and effective record keeping.
- Exceptional presentation skills with the capability to communicate effectively with different audiences in a manner which is effective and consistent with SensesWA Purpose and Commitments.
- Cert III/IV in Disability Services or other relevant qualifications will be an added advantage.
- National Disability Insurance Scheme Worker Screening Clearance in accordance with the National Disability Insurance Scheme (Practice Standards—Worker Screening) Rules 2018.
- Current and Valid WA Working with Children Check (WWCC).
- A current First Aid Certificate.
- Current WA motor vehicle driver’s licence.
