We are currently seeking an experienced Social Worker to join a well established and supportive primary Health Team in a regional Victorian hospital in Robinvale – as a sole Social Worker you will work as part of a multidisciplinary health care team within the hospital, providing quality social work services to patients/ clients of the service in order to meet their health and wellbeing needs.
Key Responsibilities
Work as a case manager, clinician and therapist with individuals, couples, families and groups as they deal with issues that arise throughout their lives/
Provide information to clients/individuals about a large range of resources and services to assist them with their social difficulties.
Plan, implement, and evaluate a broad clinical caseload ensuring client file notes, and all other correspondence are to a high and professional standard.
Complete and provide statistics to the administration staff in a timely and professional manner.
In some areas of practice, such as corrections and child protection, Social Workers make assessments of situations and may write reports with recommendations.
Use creativity and innovation when developing and delivering client interventions.
Participate and promote in community education, health promotion activities, and community wellbeing.
Ensure an understanding and respect for a range of different cultural backgrounds, beliefs, religions, and other attributes of an individual.
About You
Completed Bachelor or Masters of Social Work
Eligibility for membership of the Australian Association of Social Workers (AASW).
Demonstrated effective written, verbal and interpersonal skills.
Demonstrated ability to work as a member of a multidisciplinary team.
Demonstrated ability to effectively prioritise and organise own work / caseload across a regional area.
Demonstrated commitment to quality improvement
Demonstrated ability to provide evidence based practice relevant to clinical discipline.
Current unrestricted drivers' licence and the ability to maintain.
