Organisation:Brightside Recruitment has partnered with a growing organisation with the aim to empower, connect and support those in need within the community through providing support coordination, research projects and community events.
Role:
In this role you'll be helping individuals smash their goals by providing person-centered support coordination.
You'll get to work with clients presenting a range of conditions, with various levels of complexity in their support needs.
You'll be joining a small but supportive team of passionate Support Coordinators all focused on strengthening their client's way of life.
You will be required to:
- To assist participants with all aspects of their NDIS plan-coordination, implementation and review.
- To provide support to clients without NDIS plans in order to maximise any funding opportunities.
- To support clients and disability support workers with all aspects of service delivery.
- To work within a community context to ensure members have access to a range of supports.
- Qualifications in Social Work, Social Science, Allied Health or Nursing, a minimum of a Diploma of Community Services.
- Experience in supporting people with a range of disabilities including intellectual/developmental disability and be passionate about enabling people with intellectual disabilities to live a fulfilling and inclusive life.
- Proven experience in providing expert advice and consultation to NDIS participants and their families on the changing NDIS environment.
- Strong knowledge of the NDIS and Quality and Safeguards.
- Ability to build NDIS participant and family capacity to understand and navigate service systems.
Extra info:
- Must have a current NSW Drivers Licence
- Must have a NSW WWCC
- Must have a satisfactory National Police Check
- Must be vaccinated against Covid-19
