About the OpportunityReporting to the Community Health Programs Manager, the NDIS Support Coordinator is responsible for supporting the National Disability Insurance Scheme (NDIS) by coordinating activities that support participants to:
- Achieve goals in line with their NDIS Plan;
- Coordinate and book their NDIS supports;
- Answer any questions about the NDIS and their Plan;
- Build their ability, confidence and skills to use their NDIS Plan, including identifying, planning and advocating for services they need and making NDIS service agreements;
- Access mainstream and community services; and
- Build their capacity generally in day-to-day life.
To be considered for this position, you must have:
- Tertiary qualifications in Social Work, Psychology, Occupational Therapist and/or a minimum Certificate IV in Disability Services or similar and/or equivalent knowledge and experience within the sector;
- Knowledge and experience of the NDIS Support Coordination function and why it is important in building a Participant's capacity and to the overall success of the NDIS;
- Demonstrated understanding of the application of National Standards for Disability Services (Rights, Participation and Inclusion, Individual Outcomes, Feedback and Complaints, Service Access and Service Management);
- Previous experience with working with Aboriginal and Torres Islander people, in an NDIS context;
- Able to support and mentor staff to use initiative, work flexibly, prioritize work and to work in a person-centred way;
- Demonstrated experience using person centred approaches to effectively develop and implement individual person-centred plans;
- Relevant understanding of co-morbidities faced by the general population in the Pilbara and the Western Desert;
- Knowledge of relevant service providers across mainstream and other community service sectors, including mental health and disability providers;
- Demonstrated understanding of episodic nature of mental illness and its impact upon a person's capabilities;
- Excellent written and oral communication skills;
- Demonstrated working knowledge of MS Word, Excel and other online client reporting systems;
- Demonstrated ability and willingness to work in a competitive market environment where required to meet targets;
- Demonstrated ability to respond and adapt to competing priorities; and
- Demonstrated ability to uphold the principles of cultural safety including an ability to communicate effectively and sensitively with Aboriginal and Torres Strait Islander peoples.
- The person identifies as Aboriginal and/or Torres Strait Islander and is acknowledged as such by their community.
- Experience establishing NDIS in Aboriginal Medical Service or equivalent.
- Experience in human services - disability, mental health, housing or experience in another relevant sector.

PAMS operates five sites, four of which are remote and managed by a team of clinicians and a range of ancillary workers. Although the successful candidates will have a substantive location (Newman) they must be prepared from time to time to work across any one of these sites should they be required.
PAMS operates in some of the most remote parts of Australia and staff are required to be proficient at driving a 4WD vehicle and willing to fly in light aircraft.
About the OrganisationWith an administrative hub based in Newman, in the Pilbara region of Western Australia, The Puntukurnu Aboriginal Medical Service (PAMS) provides holistic primary health care to the individuals and families within Newman and the remote communities of Jigalong, Parnngurr, Punmu and Kunawarritji.
PAMS has over 700 registered clients, with most being Martu. Services offered include health education and promotion programs and initiatives, as well as the monitoring and management of ongoing health issues such as diabetes, hypertension, ante-natal & post-natal care and child health
