The role: Our client is actively seeking a dedicated NDIS Support Coordinator to join their team, focusing on providing comprehensive services to individuals under the age of 65 with impactful disabilities. The primary responsibility is to effectively manage and coordinate service requests, ensuring a prompt response within one business working day.
In this role, you will play a pivotal role in overseeing the delivery of services through the NDIS. This involves direct collaboration with clients throughout the NDIS process, obtaining NDIS packages, and conducting assessments to determine eligibility. Efficient coordination of client data through the NDIS provider portal and the Client Information Management System is essential, with a commitment to meeting 100% data reporting requirements.
Operating within an Active Service Model of Care, you will emphasize capacity building, wellness, and restorative care, ensuring service plans align with clients' preferences and undergo regular reviews. Compliance with organizational processes for Continuous Quality Improvement, along with adherence to Health and Safety legislation, is integral. In your position, you will actively contribute to ongoing learning, maintain up-to-date professional knowledge, and uphold our client's Code of Conduct, fostering a commitment to excellence in community and disability services.
Essential Criteria:
Bachelors or higher in Nursing (preferred)
AHPRA registration (preferred)
Clinical experience (preferred)
Minimum 2 years experience as Support Coordinator
3x COVID-19 Vaccinations
Drivers license and access to reliable and fully insured car
Police Check and NDIS Workers Screening Check
