Job Role:
We are currently seeking a proactive, vibrant and motivated Support Coordinator to join our amazing team working in Toowoomba and surrounding areas.
We are offering permanent full-time or part-time hours which can be discussed in the initial interview.
You will be responsible for coordinating supports in line with Participant's NDIS plans which support them to achieve their goals by;
Locate, coordinate and facilitate a range of supports as per identified needs
Connect Participants with mainstream supports
Track plan usage and ensure supports fit within budget constraints
Ensure supports are working toward Participant goals
Work collaboratively with Participants, informal and formal supports, providers and other stakeholders
All training provided
Experience/ Qualification/ Requirements:
Cert III in Disability or Community Services, or similar preferred, or willing to work towards
