Tarampa Assist (an arm of Tarampa Lodge) is an innovative NDIS provider offering Support Coordination to customers in the community wishing to reach their individual goals. Tarampa has been operating for over 40 years and is a family run service which prides itself on projecting a family atmosphere. The Tarampa Assist arm supports customers with an approved NDIS plan nurturing the growth of each individual at their own pace.
Support Coordinator's assist NDIS participants with the implementation of supports funded in their plan. We are looking for a dynamic thinker to works within the Somerset, Lockyer Valley and Ipswich region.
A mobile phone, laptop and reimbursement for business related travel are included as part of this role.
Job tasks and responsibilities
Role and Responsibilities:
- Assist customers with Support Coordination
- Support the participant to implement their NDIS plan
- Monitor plan outcomes and expenditure to ensure the plan remains within budget
- Strengthen and enhance the customers' abilities to self-direct their own supports and participate in the community
- Build effective relationships with customers and their families while maintaining appropriate and professional boundaries
- Ensure mainstream services meet the obligations to the customer (Housing, education, justice, health)
- Clear and accurate record keeping of information, time and billing
- Build or support the capacity of teams to develop relationships and partnerships in local communities
- Provide a detailed report to NDIS, to support the customer with their NDIS plan review meeting
- Complete and maintain organisational documentation within the scope of your role
- Work collaboratively with the Tarampa Assist team
- Respond to NDIS enquiries in a timely, professional manner
- Attend Stakeholder and Networking meetings
- Work within legislation and organisational Policy and Procedures.
Skills and experience
- Must have a Diploma in Disability or higher in Community Services or the equivalent
- Experience working with Psychosocial Disability and Acquired Brain Injury advantageous
- Hold a current QLD driver’s license
- Innovative, proactive approach to the position showing initiative and outcomes
- Have experience in a similar role
- Understanding of NDIS rules and support guidelines.
- Strong understanding of NDIS Support Coordination role and responsibilities
- Work well in a team or individually work collaboratively and collegiately to achieve optimum client outcomes.
- Must obtain a Yellow Card (National Worker Screening), Pre-employment Medical, NDIS Worker Orientation, COVID Safe Training etc.
- Clean Driving Record (Must obtain a Driver History Report Annually)
