About the Role
As an NDIS Support Coordinator, you will assist NDIS participants in coordinating and implementing a range of support services in line with their NDIS plans, using a person-centered, active support approach. You will build strong relationships with various stakeholders, including individuals with disabilities and/or mental health issues, their families, NDIS Local Area Coordinators, NDIS Planners, and both internal and external service providers.
This permanent full-time position (38 hours per week) offers some flexibility to work from home or from our Speers Point office. Some travel within the Hunter region is required, so a NSW driver's license and your own reliable vehicle are necessary.
Key Responsibilities
- Enhance clients' ability to coordinate supports and participate in the community
- Regularly monitor clients' NDIS plans to ensure services are delivered within their allocated budgets
- Ensure mainstream services (housing, education, justice, health) meet their obligations to clients
- Provide reports on outcomes and success indicators to clients and the NDIA, according to the agreed reporting frequency
Skills & Experience
- Experience working with individuals with disabilities and mental health issues
- Operational knowledge of the NDIS (3 years+)
- Ability to work in a KPI-driven environment with high motivation and commitment
- Excellent verbal communication skills and ability to engage effectively with clients and their families
- Strong written and computer skills for creating plans, reports, correspondence, and other documents
- Creative thinking and initiative
Successful candidates must clear probity checks, including National Criminal History Record Check, NDIS Workers Screening Check, and NSW Working with Children Check.
