About the role:
The Gerontology NP:
Is an expert practitioner who, through leadership, will facilitate evidence based clinical practice aimed at providing gerontology focused health care to patients with complex needs within a defined scope of practice;
Have extensive experience in gerontology and chronic disease management;
Function autonomously and collaboratively in an advanced and extended clinical role within the HIV and geriatric services;
Be grounded in the nursing profession’s values, knowledge, theories, and practice and provide innovative and flexible health care that compliments other health providers;
Provide leadership that facilitates the ongoing development of clinical practice utilizing research in the provision and development of educational programs, clinical services planning and development of clinical practice standards;
Will contribute to the advancement of quality health care systems within the department(s) to reduce avoidable hospital admission;
Will support the continuum across the emergency department, inpatient, outpatient, and community setting and is an expert on service availability;
Conduct a comprehensive geriatric assessment (CGA) where indicated using advanced knowledge and skills.
Will develop, enact, and evaluate a person-centred plan of care specific to the patient’s needs which may include but is not limited to ordering diagnostic investigations, prescribing or deprescribing medicines and direct referrals to other health practitioners and healthcare agencies in liaison with:
The patient and/or family/significant other(s);
Residential care facilities;
General practitioners;
Specialist medical officers;
The wider multidisciplinary teams including but not limited to social workers, physiotherapists, occupational therapists, dieticians;
The clinical nurse specialist for HIV complex care, the gerontology clinical nurse consultant, the emergency department Nurse Practitioners and clinical nurse consultants; and
Community health services/providers.
Conduct frailty assessments to detect frailty and pre-frail syndromes and provide evidence-based interventions to prevent or minimize frailty, enhance quality of life, and decrease the need for inpatient care;
Using a case management model and in collaboration with the CNS for HIV complex care and the HIV social work team, provide support and care for patients living with HIV with complex bio-psychosocial needs promoting medication adherence, engagement in care, harm minimization and service(s) navigation;
Provide early assessment of geriatric patients in the emergency department to begin the care and discharge process before formal admission;

Selection Criteria:
1.Personal integrity and demonstrated commitment to the Philosophy, Mission and Values of Mary Aikenhead Ministries.
2.Currently registered as a registered nurse with the Australian Health Professionals Regulation Agency (APHRA) and endorsed by the Nursing and Midwifery Board of Australia to practice as a Nurse Practitioner.
3.Advanced understanding of workplace health and safety.
4.Advanced understanding of clinical governance and current nursing issues. 5.Demonstrated high level leadership within the workplace and across external agencies including the ability to form productive working relationships through highly developed communication, negotiation, facilitation, and conflict resolution skills.
6.Comprehensive understanding of clinical governance, clinical practice development and current nursing issues.
7.Demonstrated experience in contribution to nursing research, quality improvement initiatives and policy and procedure development and implementation. Computer literacy in MS Word/Excel, email and electronic medical records/referral systems
