What you'll be doing
The Nurse Practitioner/ Transitional Nurse Practitioner Aged Care will function autonomously and collaboratively to provide a high level of clinically focused nursing care, practicing within their regulated scope of practice, across diverse practice settings and patient/client populations within a defined specialty.
The-NP will contribute to the advancement of quality health care systems within the Local Health District and across Residential Aged Care Facilities (RACF's) to reduce avoidable hospital presentations and promote early discharge from hospital.
To manage care across a variety of health systems to maximize outcomes - engaging in complex and critical thinking; integrating information and/or evidence; judiciously use clinical investigations; skilfully and empathetically communicating with all involved in the care episode, including the person receiving care and their family and community, and health professional colleagues.
To engage in practice as guided by the Nursing and Midwifery Board of Australia Nurse Practitioner Standards Framework.
All NSW Health workers are required to have completed a primary course (2 doses) of a Therapeutics Goods Administration (TGA) approved or recognised COVID-19 vaccine (except for the Janssen COVID-19 vaccine, which is approved by the TGA as a single dose primary course).
New applicants must have completed the vaccination course prior to commencement with NSW Health or provide an Australian Immunisation Register (AIR) immunisation history statement certifying the worker cannot have any approved COVID-19 vaccines available in NSW. A NSW Health agency may require further information about the medical contraindication (including but not limited to an Immunisation Medical Exemption form – IM011 form).
Acceptable proof of vaccination is the Australian Immunisation Register (AIR) Immunisation History Statement or AIR COVID-19 Digital Certificate. Booster doses are highly recommended for all health care workers who have completed the primary course of COVID-19 vaccinations.

- Current registration with the Nursing and Midwifery Board of Australia with endorsement to practice as a Nurse Practitioner or currently working toward endorsement as a Nurse Practitioner and ability to complete requirements within a 3-year period.
- At least 3 years full time equivalent post registration nursing experience with extensive clinical experience in Aged Care/Psychogeriatrics and ability to function at an advanced autonomous level.
- A post graduate qualification relevant to the clinical specialty -Aged Care / Psychogeriatrics.
- Demonstrated comprehensive skills in clinical assessment including physical, mental health, social, cultural, diagnostics and prescribing decisions.
- Excellent leadership and interpersonal skills with proven ability to use appropriate teaching/learning strategies to educate and support others within the specialty and the profession.
- Proven ability to critically evaluate and integrate relevant research findings into decision making about health care management and interventions.
- Evidence of implementing interventions and monitoring outcomes to evaluate and improve patient care.
- Current NSW driver's license and ability and willingness to travel for work purposes.
