About the Opportunity
- Providing a high standard of clinical services to a range of age groups that includes client-focused assessment and therapy, equipment prescription, and home modifications.
- Develop care plans and monitor adherence to care plan, evaluate effectiveness, monitors client’s progress in a timely manner and facilitate any changes required.
- Ensure the efficient and effective use of resources, including setting priorities so those clients with the highest needs are given priority in the provision services.
- Work closely with the multi-disciplinary team and senior management to support improved care planning and continuity of care across service boundaries.
- Coordinate the promotion of Primary Care support services and resources to relevant stakeholders.
Selection Criteria
- Professional Registration AHPRA
- Experience in similar role is desirable
- Knowledge of disability & mental health services.
- Driver’s Licence
- NDIS Worker Screening Check
- WWCC
