It is an ongoing, full time opportunity to join our large, friendly community-based team. You will be working across a variety of specialties and client conditions including (but not limited to): Orthopaedic, Neurological, Pain management, Gerontology and Cardiorespiratory. You will deliver individual and group physiotherapy services within a community centre, clients' home environment, and community settings. Grade One’s looking for the next step in their career encouraged to apply!About UsThe Aged and Community Care (ACC) Program provides an interdisciplinary service to support people over the age of 65 in the community. The program aims to keep the client independent within their community, proactively promoting health and independence, and self-management whilst assisting with timely interventions to prevent deterioration of the condition, and admission to hospital or residential care. We offer a client-centred, goal directed approach to a growing and diverse population.Transition Care Program (TCP) aims to minimise the number of older people experiencing inappropriate extended hospital lengths of stay, being prematurely admitted to residential care and/or prevent unnecessary presentations or admission to the Emergency Department. The program provides short term support and active management for older people at the interface of the acute/subacute and aged care (i.e. community and residential) sector. The program is goal orientated to optimise the patient’s functional capacity and finalise and access their longer term care arrangements or maintenance at home. TCP can be provided as a bed based or as a home-based service.What you need:
  • Bachelor of Physiotherapy (or equivalent qualification),
  • Current registration with the Australian Health Practitioner Regulation Agency (AHPRA) – Physiotherapy Board of Australia (practicing, no restrictions)
  • Current Driver’s License
