About the role:
Our Physiotherapist takes the lead in managing our outpatient rehabilitation gym Monday to Friday mornings. Supported by our Team Leader, Allied Health Assistant, Enrolled Nurse, and other members of the interdisciplinary team.
After preliminary individualised rehabilitation assessment appropriate patients attend a program of rehabilitation in our outpatient gym, their own home, community settings or via telehealth. Location of care is based on patient centred goals. Patients work on individualised programs in a group setting or on individualised programs at home, community, and in telehealth.
Afternoon sessions can be completed 1:1 in our outpatient gym, home, community environments and via telehealth as relevant to patient goals. Our Physiotherapists support patients to translate rehabilitation exercises into function within the community. This may include supporting patients to transition to community-based exercise programs.
Our Physiotherapist collects and collates standardised and patient-centred outcome measures. This data informs quality improvement processes to further improve patient-centred care.
In this role our Physiotherapist takes on several interdisciplinary roles with the Allied Health Team, Interdisciplinary intake assessments, case management and coordination, complex report writing and team and administration discharge planning.

- Westlakes Community Rehabilitation Team provides care to a diverse, interesting range of patients. Populations include subacute stroke. multi-morbidity, frailty and falls and neuro-degenerative illnesses.
- This is a solo physiotherapy position in an interdisciplinary team. Our Physiotherapist works with our Team Leader, Speech Pathologist, Social Worker, Occupational Therapist, Enrolled Nurse and Allied Health Assistant.

- In this role you will be supported by the Team Leader. Professional support will be provided by the Community Physiotherapy Head Of Discipline. The Physiotherapist will be supported to attend Community Physiotherapy meetings and professional development opportunities.
- The Physiotherapist will be provided with support to develop specialist skills in behaviour change, falls prevention, subacute rehabilitation and care of patients with advanced neurological disease.
- Westlakes Community Rehabilitation has a well-developed orientation system. Written administration guides and peer support and supervision will be available to orientate to this role.
Requirements:Our successful candidate will possess the following:
- Physiotherapist with current Authority to Practice with AHPRA.
