The Virtual Rehabilitation Ward is dedicated to providing high-quality care to individuals with complex medical, functional, and psychosocial needs in the comfort of their own homes, including residential aged care settings. Our team delivers multidisciplinary assessment, goal setting, rehabilitation interventions, and sustainable care planning in close collaboration with the individual, their family, carer, and existing community services.
About the Role:
As a Physiotherapist within our team, you will play a vital role in the provision of holistic care to our clients. Key responsibilities include:

• Conducting assessments and developing tailored treatment plans for clients;
• Providing evidence-based physiotherapy interventions to optimise functional outcomes;
• Collaborating with clients, families, carers, and community services to establish sustainable care plans;
• Integrating digital strategies such as Telehealth and home-based monitoring into care provision; and
• Working closely with the Home Rehabilitation Team and general practice teams to ensure seamless care coordination.

To be successful in this role, you will have:

• Ability to undertake assessment and intervention with clients with a range of illnesses and disabilities relevant to older people and rehabilitation.
• Ability to work collaboratively within a multidisciplinary team.
• Flexibility to work 7 days a week as required by the service.
• Strong interpersonal skills with the ability to work collaboratively within a multidisciplinary team.

Experience in Virtual Rehabilitation, telehealth, and home-based care is highly desirable. Please see the attached Role Description for additional information.
