About the Role
An opportunity has opened for a part time Registered Nurse to step in and provide dedicated coordination of Chronic Disease Management (CDM), Health Assessment (HA), and ATSI Assessments. This nurse will be responsible for developing and monitoring systems that support the clinic with delivery of consistent, efficient, and effective Care Coordination. This role will have a focus on meeting patient healthcare needs and General Practitioner (GP) requirements and ensuring the most appropriate utilization of the Medicare Benefits Schedule. Our ideal candidate will have a strong work ethic, a caring nature, high level communication skills and attention to detail. There is opportunity for this role to increase from part time to full time.

Key roles and responsibilities will include:
- Develop and support the implementation of a Chronic Disease Management process for eligible patients, coordinating care with internal team members and external health care providers
- Monitor the performance of CDM processes, with support from the Practice Manager
- Delivery of GP Management Plans, Team Care Arrangements, Health Assessments, CVC program, ECGs, Spirometry, urine testing, diabetes cycle of care, Asthma Management Plan, Comprehensive Medical Assessments
- Participate in Quality Improvement Activities
- Provide culturally appropriate assessments, therapeutic and education services for patients with chronic disease as required
- Ensure that patients receive education on the use of therapies and treatments prescribed and feedback on the outcomes of therapy and treatment
- Adhere to relevant professional Code of Ethics guidelines
- A willingness to implement and adapt to change when required to benefit patients, staff, and the wider group
- Provide general nursing support to General Practitioners
Required skills and experience:
- Previous General Practice experience essential
- Experience with CDM, HA, and ATSI
- Strong computer literacy
