The Derbarl Yerrigan Health Service (DYHS) was established in 1974 and is the largest Aboriginal community-controlled health service provider in Western Australia, providing care to over 20,000 Aboriginal patients across four clinics in East Perth, Mirrabooka, Maddington and Midland. Our staff enable the delivery of culturally responsive primary health services, focusing on prevention, early intervention, comprehensive care and care coordination including chronic disease management.

Job Summary

The Project Coordinator role has been designed specifically for the Journey Together Initiative Project which is in collaboration between the Telethon Kids Institute (TKI) and Derbarl Yerrigan Health Service (Derbarl). This role will coordinate specifically the Family Needs Assessment Phase of the project. The role will work closely with Derbarl General Practitioners, the Derbarl Paeds team, the general Derbarl primary health care team and the TKI Research team. This role will recruit patients and families to take part in Aboriginal health assessments with the GP and to work closely with the TKI research team including regular contact and updates to ensure the various research components are completed. This will be a Maximum term part-time contract (0.8 FTE) of 6 months from start date.

Key Responsibilities

The Project Coordinator will oversee the following functions:

  • Assist with recruiting of patients and families.
  • The recruitment target is 60 households (at least one adult and one child per household).
  • To assist the GP to perform Aboriginal health assessments as per the research project design.
  • Ensure informed consent is done with patients and families to participate in the project.
  • Regularly meet with the TKI Research team to provide updates and ensure the project timelines are being adhered to.
  • Ensure the integrity of patient management systems and records are maintained.
  • Assist the GP with follow up and recall of patients and families as required if issues are identified during or post health assessment.
  • Prepare and coordinate reports to TKI Research team, ensuring all reporting requirements are met in a timely and accurate manner.
  • Practice duty of care including meeting practice standards and accountability.
  • Undertake clinical duties in own area of qualification to provide a high level of case coordination to complex clients where required by the team.
  • Implement & monitor client patient privacy policy.
  • Communicate effectively with all staff and patients using the appropriate channels, utilising appropriate formal and informal channels of communication.

SELECTION CRITERIA

Essential

  • Hold a recognised health associated post graduate qualification and be eligible for practicing membership of the relevant professional organisation.
  • Demonstrated project management skills especially if used within a research project.
  • Demonstrated clinical skills and knowledge in planning and case coordination
  • Demonstrated well-developed organisational and time management skills including written and verbal communication
  • An ability to communicate sensitively in a cross-cultural environment with Aboriginal clients and staff.
  • A demonstrated understanding of the unique issues affecting and impacting on the health and wellbeing of Aboriginal children and their families.
  • Demonstrated understanding and commitment to the principles of the Aboriginal Community Controlled Health Sector and demonstrated history of working with Aboriginal People.
  • Current Drivers licence.

Desirable

  • Aboriginal and or Torres Strait Islander peoples – Section 51D of the Equal Opportunity Act.
  • Knowledge of the community and local Aboriginal cultural issues.
  • Demonstrated clinical knowledge and experience working with disadvantaged families and child health.
  • A good working knowledge and expertise in the use of Communicare or a similar patient information & recall system.

Appointment Pre-requisites

  • Provision of the minimum identity proofing requirements
  • Successful Criminal Record Screening Clearance
  • Successful Pre-Employment Health Assessment
  • Successful Working with Children Check
  • Evidence of a current C or C-A Class driver’s licence or other specialised licence class
