Regional Assessment Service (RAS) - Home Support Assessor - what's involved in the role?

We are seeking a Full Time RAS Home Support Assessor on a maximum term contract ending 31st December 2024, to join our team and assess our customers located on the Sunshine Coast. You will have the ability to book your own schedule to allow a great work life balance. In this role, you will be provided with specific training in the assessment process through an online platform called My Aged Care Learning Environment or MACLE. This training is paid for by Suncare.

In the role of a RAS Home Support Assessor, you will use the My Aged Care, NSAF (National Screening Assessment Form) to undertake a face to face, in home assessment that will identify areas of risk or required support. You are then able to assist the person to find solutions that will enable them to retain their independence by connecting them with relevant service providers and local support.  Important to the role is the ability to quickly build rapport, to communicate effectively with a range of people and to identify the support required, so that the person can continue to live a full and independent life.  

About you:

The successful candidate will have a tertiary qualification in Social/Human Services or related field and you will have experience in individualised service planning, referral and care co-ordination and a demonstrated knowledge of community sector support services. We want our customers to have a great experience with our people and our people need to have a great attitude, be friendly, have excellent organisational skills, be digitally literate, be willing to learn new things, be able to work autonomously and most importantly listen and understand what the customer wants and needs. Experience in conducting aged care customer assessments and experience in creating support plan is highly desirable.

For this position you will be required to have a valid Driver's licence and be willing to use your own vehicle for work purposes, ensuring that your vehicle is appropriately insured. You are required to possess a CPR Certificate and National Police Check but if you do not hold these currently, we can assist you in completing these requirements as part of the recruitment/onboarding process.
