Position Profile: We are seeking to appoint a suitably qualified and motivated individual to join our Community Health team based in Kalgoorlie. Reporting to the Clinical Nurse Manager-Community Health and working under the Principles of Primary Health Care you will be part of a supportive Population Health team responsible for the provision of quality care to clients, families and the community. You will work alongside other members of the Population Health team including Clinical Nurses, Clinical Nurse Specialist and Staff Development Nurse.
