About the role
The Registered NurseIntegrated Care Coordinator will operate within a dynamic, supportive district wide Integrated Care in the community team, collaborating with clients, carers, General Practitioners and service providers across all sectors of health and social care.
Your role will involve delivering comprehensive holistic assessments, connected and coordinated care to people identified for Integrated Care programs operating under Western Virtual Service Delivery Directorate.
