What you will be doing:
The role of the Registered Nurse within Aboriginal Chronic Care Program Outreach Service is to provide a mobile Outreach Service to Aboriginal people at locations identified by the Community that screens, educates and coordinates integrated care strategies including referral to appropriate services and psychosocial assessment. The Registered Nurse will work in partnership with the Aboriginal Health Workers and other services within NBMLHD to address the needs of the community and support non-Aboriginal staff in developing their cultural competency. The Registered Nurse will also assist in the service reporting and evaluation and other tasks as directed by the Director Aboriginal Health. In this role you will be responsible for the provision of a culturally responsive, mobile Outreach Service to the Aboriginal communities in various locations across Nepean Blue Mountains Local Health District.

All NSW Health workers are required to have completed a primary course (2 doses) of a Therapeutic Goods Administration (TGA) approved or recognised COVID-19 vaccine (except for the Janssen COVID-19 vaccine which is approved by the TGA as a single dose primary course). New applicants must have completed the vaccination course prior to commencement with NSW Health or provide an Australian Immunisation Register (AIR) Immunisation History Statement certifying the worker cannot have any approved COVID-19 vaccines available in NSW. A NSW Health agency may require further information about the medical contraindication (including but not limited to an Immunisation Medical Exemption form – IM011 form).
Acceptable proof of vaccination is the Australian Immunisation Register (AIR) Immunisation History Statement or AIR COVID-19 Digital Certificate. Booster doses are highly recommended for all health care workers who have completed the primary course of COVID-19 vaccinations.
Employment of a temporary visa holder may only occur if no suitable permanent resident or citizen of Australia has been identified and must not exceed the duration or conditions associated with the current visa.
Selection criteria to be addressed: * Current Authority to Practice as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA).

• Demonstrated effective communication through the use of information technology and sound written, verbal and interpersonal skills
• Demonstrated knowledge of chronic disease prevention and self-management principles.
• Aboriginal cultural awareness and sensitivity with knowledge of the issues affecting the health of Aboriginal people.
• Demonstrated ability and experience in building and maintaining positive partnerships and networks for delivering health services
• Demonstrated understanding and commitment to quality improvement principles.
• Valid current Drivers License and willingness to use for work purposes
